An Internet exchange point (IXP) is a physical location through which Internet infrastructure companies such as Internet Service Providers (ISPs) and CDN s connect with each other. These locations exist on the “edge” of different networks, and allow network providers to share transit outside their own network.

The last mile is the segment that connects a local ISP to a customer, such as via a cable line to the home. The type of technology used to deliver last-mile service is the primary determinant of the speed and quality of service available to consumers.
A regional ISP will have a few nodes in a county/state, but they'll probably buy their [inter]national transit from a tier 2 (ie: they're transit heavy). A tier 2 tends to have a large network, and has a heavier reliance on peering (remember: peering is not free, just a little cheaper at scale), however they still have transit needs.
